I have installed the SUA subsystem in 2003r2 using the install new programs applet.

I then subsequently installed the SDK and Utility programs, but the install
contains no option under custom to change the drive letter.

There is just no option shown at all for the base product or the individual items.

Can the SDK be installed to other than the default drive c: under the Windows directory (C:\Windows\SUA)?

With SUA (aka Interix) being part of the base OS release starting
with W2K3/R2 Microsoft puts all of the base OS programs in the same
location.

I haven't tried to put it anywhere else. However, it might be possible
with an unattended installation (not using the MSI GUI). There's a white
paper on the MS web site for unattended installation for SFU 3.0. The same
paper is mostly applicable to 3.5. So it might work with the same options.
